Arrival
When you and your family or friends arrive at the surgery center, please check in at the front desk where we will register you.
We will verify:
- Your insurance
- Payment information
- Copy a photo ID and proof of address (If patient is a minor, photo ID and proof of address of a parent or guardian) will be verified and copied.
