Forms to return to ISS before your surgery
In your patient folder, you will find a yellow, green, and blue form that you will need to fill out and return to ISS in the stamped envelope included in the folder. If it is less than three (3) days until your surgery, we would recommend that you bring the filled-out forms with you the day of surgery. Samples of the forms that you could fill out online and print, then either mail in or hand carry the day of surgery are on this website.
